Insurance Forms:
If you need insurance forms filled out, send them to us at forms@orthopedicassociates.ca. Please include your name, birthday, and phone number.
Alternatively, you can drop them off to the office in person. See our office hours above. Please note there is a personal charge to the patient for all forms. We will notify you of the charge after you submit your forms. We take payment via credit card or debit. We can take credit card over the phone and email a receipt. Please allow up to 2 weeks for completion of forms.
Work Notes:
If you need a work note from your surgeon, send a request to worknotes@orthopedicassociates.ca. Please include your name, birthday, and phone number.
